Job Title: Adolescent Care Coordinator

Reports To: Adolescent Program Manager

Compensation: $18-23/hour

Status: Full-Time

Shift: 8:30-4:30pm Mon-Fri

Imagine by Northpoint has an opening for an Adolescent Care Coordinator in our Lincoln, NE adolescent outpatient facility.

POSITION SUMMARY: As a key member of the Imagine by Northpoint team, the Adolescent Care Coordinator I is responsible for coordinating and providing transportation needs, maintenance and record keeping for company vehicles, supervision and monitoring of adolescents during transportation and programming, case management support, crisis intervention, coordination of care and routine administrative support. This position will interact with leaders, employees, clients, physicians, and will work closely with the administrative team. This ideal incumbent will operate with a high level of energy, resourcefulness, enthusiasm, and work ethic.

ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

  • Oversee and manage the safe and efficient transport of adolescents to and from our facilities
  • Direct and monitor daily assignments – schedule drivers and riders, and assign vehicles to ensure daily coverage of all routes and field trips
  • Maintain route documentation and records (relevant contract requirements, patient data, route changes, relevant maps, vehicle details, and driver details) for the purpose of providing accurate and reliable information to the Lead Counselor
  • Monitor the safety and cleanliness of company vehicles by washing vans inside and out when needed, and sweep, clean, and sanitize the vehicle after completion of each route
  • Perform pre and post trip vehicle inspections and arrange for the repair of any vehicles as the need arises
  • Develop and follow a preventive maintenance schedule for each vehicle
  • Maintain gas receipts and complete the necessary paperwork for payment of fuel vendors in coordination with the Accounts Payable team
  • Assist clients with scheduling community appointments
  • Assist clients with schooling needs and collaborate with school appointed resource
  • Provide facilitation and co-facilitation of approved group sessions
  • Assist in orientation of new admits
  • Assist with updates to treatment plans
  • Assist with the required documentation of group sessions
  • Interact routinely with clients, observe behaviors and communicate significant observations to clinical staff
  • Document information regarding client care and behavior as required in the policies and procedures
  • Utilize crisis prevention intervention when exposed to volatile patients to maintain personal safety while also ensuring safety for the patients and other individuals on the unit
  • Assist in admissions and discharges of patients as assigned
  • Assist nursing and counseling staff in responding to emergency situations
  • Maintain confidentiality in accordance with established policies and procedures and standards of care
  • Perform other job-related duties as assigned

QUALIFICATIONS/REQUIREMENTS FOR POSITION:

  • Any combination of education and work experience equivalent to a high school diploma and two years of work experience with adolescents 12-17 years old struggling with behavioral and mental health conditions
  • Bachelor’s Degree in relevant field, preferred
  • Must be at least 21 years of age
  • Knowledge and skills in English usage, interpersonal relationships and excellent communication skills
  • Must be able to monitor regular maintenance (oil changes, tire rotations, etc.)
  • Must possess the ability to make independent decisions when circumstances warrant such action
  • Must be knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in a dual diagnosis adolescent program
  • Must have the ability to implement and interpret the programs, goals, objectives, policies, and procedures of the department
  • Knowledge of surrounding geographic area to ensure effective and efficient routing
  • Ability to organize and prioritize time to balance conflicting demands
  • Must perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation
  • Must understand and follow company policies including harassment and compliance procedures
  • Display integrity and professionalism and adhere to all Company policies and procedures

  • Excellent communication skills; ability to communicate clearly and concisely, verbally and in writing

  • Ability to interact and communicate with individuals at all levels of organization

  • Ability to effectively prioritize workload in a fast-paced environment
  • Proficiency with Microsoft Office Suite

CONDITIONS OF EMPLOYMENT:

  • Completion of a tuberculin screening during first week of employment
  • Completion of a pre-employment drug screening, post-accident and upon reasonable suspicion of use
  • Completion of background and reference checks
  • Completion of a Motor Vehicle Record (MVR) check
  • Must be fully vaccinated for COVID-19
