Job Title: Adolescent Care Coordinator

Reports To: Adolescent Program Manager

Northpoint has an opening for a Care Coordinator in our Seattle, WA outpatient facility.

POSITION SUMMARY: As a member of the Imagine Team, the Adolescent Care Coordinator is responsible for helping clients and families secure resources and community services, co-facilitiate group, and assist with schooling needs. The Adolescent Care Coordinator will work under the direction of the counseling staff, leadership, and multidisciplinary team to coordinate care for all clients within the adolescent program.

ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

  • Coordinate and manage breakfast, lunch, and snack breaks for clients
  • Interact routinely with clients, observe behaviors and communicate significant observations to clinical staff
  • Document information regarding client care and behavior as required in the policies and procedures
  • Utilize crisis prevention intervention when exposed to volatile patients to maintain personal safety while also ensuring safety for the patients and other individuals on the unit
  • Participate in maintenance of a clean, orderly and safe environment, and report needs for repairs and maintenance to appropriate staff
  • Assist in admissions and discharges of patients as assigned
  • Assist nursing and counseling staff in responding to emergency situations
  • Conduct and assist with selected activity groups as assigned and document according to policies and procedures
  • Assist clients with scheduling community appointments
  • Assist clients with schooling needs and collaborate with school appointed resource
  • Provide facilitation and co-facilitation of approved group sessions
  • Assist in orientation of new admits
  • Create and assist in updating treatment plans
  • Create educational curriculum as directed
  • Participate in all trainings with the clinical team
  • Assist in documenting group sessions
  • Provide resourcing for crisis intervention (safety plans, contacting support, risk assessments, etc)
  • Assist clients in maintaining attendance to the program
  • Administrative duties that support the clinical team
  • Adhere to all company policies and procedures
  • Maintain confidentiality in accordance with established policies and procedures and standards of care
  • Other clinically appropriate services and special projects as assigned

QUALIFICATIONS/REQUIREMENTS FOR POSITION:

  • Bachelor’s degree from an accredited university in relevant field preferred
  • 1-2 years working experience with adolescents ages 13-17 years old
  • Must be at least 21 years of age

  • De-escalation and crisis intervention techniques
  • Behavioral health experience
  • Excellent organizational abilities
  • Excellent written and oral communication skills
  • Attention to detail and accuracy
  • Client service oriented (both internal and external)
  • Creative and persistent problem solver
  • Able to handle confidential material in a reliable manner
  • Ability to interact and communicate with individuals at all levels of organization
  • Strong interpersonal skills to handle sensitive situations and confidential information
  • Ability to multi-task and prioritize workload in a fast-paced environment
  • Proficiency with Microsoft Office Suite
  • Seeking a relevant licensure such as a SUDPT, SUDP, CADC, ACADC, LPC, LCPC, LMSW, LCSW or LMFT

REQUIRED CERTIFICATES/LICENSES/REGISTRATIONS:

  • Must maintain current nationally accredited CPR/First Aid/AED certification

CONDITIONS OF EMPLOYMENT:

  • Completion of a tuberculin screening during first week of employment
  • Completion of a pre-employment drug screening, post-accident and random for upon reasonable suspicion of use
  • Completion of orientation and required paperwork prior to reporting to work
  • Demonstrated computer literacy
  • Completion of CPR/First Aid Training within first 90 days of employment
  • Attendance at all mandatory staff development and training
  • Completion of background and references checks

Job Type: Full-time

Pay: $26.00-$32.00 an hour
